1-You have a medical prescription

2-You pick up your sampling kit from a partner pharmacy, based on your prescription

3-You collect the sample at home, following the instructions for use provided in the kit

4-You fill in the information form provided in the kit making sure to include your contact details

5-You place the sample, the completed information form, and the medical prescription in the pouch

6-You return your complete sampling kit to the partner pharmacy, respecting the last drop-off time.
